Recently, I made two attempts to evoke Dantalion to visible appearance. I used a mostly by-the-book approach. The main deviations from the instructions as set out in the Lemegeton were:
1. I did not have a lion-skin belt. Instead, I used yellow-dyed fabric. 2. I did not have the ring. 3. The metal box was not black. It was a stainless steel lunch box. 4. I did not have sulphur. I used only asafoetida in the metal box. 5. I did not have a sword, a wand, or a miter. 6. The first time, I did not pay attention to Dantalion's office hour. The second time, the ritual was performed during Dantalion's office hour.
Unfortunately, I was unable to see any sign of Dantalion being present. I ended up using the metal-box procedure each time.
Since you take a by-the-book approach, I would say try to stay true to the instructions even more. That is to say, get a sword, a wand, a miter, get the sulphur, get a black box.
A lot of grimoire magicians say that one doesn't need to have their astral senses open if they do not omit the instructions. So see if you'll get any better results when you don't omit anything, or at least omit very very little.
If that does not work, then keep evoking. Try another spirit. The problem may lie in the fact that your astral senses are not developed sufficiently.
Also, you only did this two times. Rarely, people are successful on their first try. You know, you can try this method as you are doing a few more times. Up to 5 times, then start getting the tools you are missing if the results are still screwy.
1. To my understanding, incense should be offered to the spirit along with the Address, after the spirit has provided a sign of its presence. As such, I placed the incense on the working table inside the circle.
I understand that some people put the incense inside the triangle. Which is the correct way of doing things?
2. If I used the metal-box procedure, e.g. the Greater Curse, should I still use the License to Depart? It seems a bit contradictory. E.g., One uses the Greater Curse, when the spirit has not appeared; however, the License to Depart presupposes that the spirit has appeared, and would appear, based on the wording, to have the effect of freeing the spirit from the curse.
I note that I did in fact utter the License to Depart during my experiments.
3. How should I dispose of the metal-box, in the event that I have used the Greater Curse? Would I free the spirit (and allow it to harm me) by opening the box?
4. Is it permissible to clean the spirit's metal seal with holy water, or is it distasteful to the spirit?
Altho I am far from a traditionalist in terms of Goetic Practice. I did study Grimoir tradition alongside when developing my own system and have much respect for it. May I ask what specific incantations you used for the ceremony??
What circle you used and what Grimoir you used for reference? And the kind of incense you used? I always saw traditional evocation as a system that pays off when extreme attention to detail is exercised.
I am not really a fan of using threats to entice entities but I subscribe in the very least the concepts of the practice. The idea of cursing them giving license to depart seemed a bit off even for me lol.
But the black box works wonders in other curse work. Just make sure the box is black and has a lid. You can get away with a small safety deposit box as well with a lock.
My advise/words of comfort is, do not stop, keep going. As Crowley summed up the key to Success is to Invoke often (I assume this also applies to Evocation)
As a followup, perhaps you can also outline the procedure you used so that we can make suggestions as well as comments. The issue could lie elsewhere such as your preparation for the ceremony. Some traditionalists claim a cleansing procedure is needed or even a call out to the ruling decans of the Entity you are trying to Summon.
Before I perform the ceremony, I wash the floor with holy water mixed with hyssop and fumigate the room with sage. I wonder whether the salt in the holy water may be preventing the spirit from manifesting.

I have a suspicion that Daemonolators don't really work with the spirits they think they're working with. But rather egregores. I can't explain it, but there's something terribly off-putting about those sigils (Dukante sigils) and the enns. Whenever I used them, they were either really weak or ineffective.
I also think that the modern approach is not really good. The best approach is the old approach: erecting altars, shrines, making idols, offerings, sacrifices, etc. Modern approach from what I have seen can often lead to self-delusion. Then again, it really depends on what we mean when we say modern approach. To me, the modern approach is the psychologization (spirits are a part of self) or energization (spirits are just energy) of the spiritual, not leaving offerings / paying the spirits, not testing spirits, etc.
This is a real risk with the modern approach. Perhaps not even Egregores but trickster spirits can also manifest.
My approach is one of worship which is fine for me and gives me good reason to verify entities presences. Fill your magic space with their imagery, make icons of worship, give offerings without asking for anything in a ceremonious fashion and invoke similar to praying to establish a connection.
Literally I take a page out of the catholic church. Each mass is really an invocation ceremony for YHVH and I do the same but with the Goetic entities I choose to pray to.
When I do invoke, I follow a similar approach as to the guides I posted.
Same here. I also do an evocation method which is inspired by 2 particular grimoires. It has a Satanic slant to it, because I don't feel comfortable with the godname of the grimoire method(s), and taking away the free will of a spirit. I understand some spirits are not willing to cooperate, but I personally stay away from such spirits. Don't want to help? Fine, I won't force you. I don't want to go out of my way to enslave a spirit. I much rather do my own dirtywork, unless the circumstances are such that I need an intelligence (e.g. a spirit) instead of raw energy.
That said, if a spirit attacked me, I will defend myself.
I think the satanic element (not to sound too edgy) is really just a show of dedication to the entities you plan on working with. In my experience, I experienced better results once I literally did some sacrilegious acts against the Church.

I too do not like the idea of subjugating spirits using the names of god as its too similar to how I felt about god subjugating me (long story).
